How about they just let the insurance companies bid for the damn program. It works for the military contracts 
Since a majority of the 30-40 million uninsured leagel citizens that do not have coverage opt out becuase it is expensive and they are general healthy individuals, it would not cost the insurance companies that much to offer an affordable base line or alacarte program, some do but they are few and far between. But hey they are out for the money and offering a reasonable program is not in their best interest.
The probalem with the interstate purchase option in the reformers programs is that you have no guarantee that the doctors in your area will accept the insurance provider from another state.
Also, insurance companies could care less about the people they cover. If they did they would actually push prevention instead of intervention, but hey, there is no money in that

That sounds very much like me, but my leg did not get that bad, it has maintained a 4" ulcer though.
